..Ok im just reading my saunders book (again), the first time i skipped the first few chapters on "how to take the nclex", i just read the "content" stuff (which is alot, the book is HUGE)
anyway, it says when answering questions on NCLEX use MASLOW for priority. i got one of the sample questions wrong becasue i put SAFETY first..we were CONSTANTLY drilled in nursing school safety first. but according to the NCLEX its second ??? maslow "physiologic" needs are first ?-

1. What would be the priority for a nurse to take when he/she sees a malnourished homeless man in the street with oncoming traffic coming his way.
A. counsel the man on his behaviour
B. call the Doctor (sorry, couldnt help myself there)
C. Offer him a ham sandwhich to eat
D. Move him out of oncoming traffic
--well, i guess im just gonna offer him a ham sandwhich then. cause MASLOW says physiological need comes before safety ??!! (huh)
sooo according to that- i now know that i missed alot of those priority questions on the NCLEX..cause i was taught pt safety first.

Who says NCLEX wants maslow?? Your "joking questions" don't make sense & don't prove a point. NCLEX wants the choice that will keep your pt alive long enough to worry about eating, etc... you've been given valuable advice by many people... take it or leave it... still wishing you the best!!

mockery of the nclex-rn will lead you nowhere.
sometimes, you should use common sense because if you employ the maslow's hierarchy of needs, it still involves choosing the best thing to keep the patient alive or best thing to improve the patient's condition.
tha's why it's important to know your nursing knowledge in 4 years of nursing school.
i passed it last month for the first time with only 75 questions. i didn't think out of the box and i just focused on what the priority nursing action the question is asking, which does not involve safety at all cases.
let us study this question i formulated myself:
a manic client paces around the unit for 2 hours. he is coolm diaphoretic and seems wobbling. the priority nursing intervention is to:
1. give the client a tumbler of orange juice and crackers(physiologic)
2. calm the patient down (safety)
3. tell the petient that he still looks like herself as a famous actress (self-esteem)
4. give the patient something to do (diversion)
okay, did you note the question? what do you think is the problem? is it his pacing which may lead to his injury or his "cool, diaphoretic and seems wobbling" problem?
i bet with your thinking, you would say the answer is number two. but it is not! maslow says physiologic first, right? so with the above problem "cool, diaphoretic...", the problem is physiologic, not safety because the patient is actually experiencing hypoglycemia! i just "unlocked" what the question is really trying to ask.
so the correct answer is number one...this addresses physio question.
if you would just try to analyze first the problem, you will be able to unlock what the question asks and you will be able to get the right answers.

..i wish i could post the question from the book,12 RN's..14 Rn NCLEX test takers and myself all missed it because we went with "pt safety"---we all went "huh", was it a bad question ? sure, are there badly written/thought up nursing/nclex questrions ? absolutely. is it reflective of ALL nclex questions ? no...am i naive enough to beleive that all nclex quesions are badly written/thought up ?..no
we have ALL had questions , nursing questions in school and on the nclex where we went "huh", ? my point is not to not to stir up hostilities, nor mock the nclex..its just to show that the nclex is not the definitive standard of what good nurses should be, i.e if you pass it -you are and will be a good nurse, if you fail-you will be a bad nurse or lack "critical thinking" skills.
i appreciate all the advice , help and support-my intentions are not to be defensive nor insulting. just posting my thoughts on a forum, to start a dialogue but mostly just to blow off some stream. some people have Im'd me and get it, others do not.

Of course we are talking about nclex and if the homeless guy is amlnourished giving him something to eat will be the nclex way to answer the question. However, as we all know and are smart enough...we will remove him from the dangers of the cars and then give him the food.
This is clearly what confuses many people. Real world versus nclex world. I believe what jon is trying to convey is why can't nclex just ask questions and the answers based on the real world...is that so freaking hard to do???
Here is another question that may boggle your mind...
You are in a pshy ward and you are taking care of pshy patients. You see a patient running down the hall screaming with scissors in his hands. What is the priority action?
1...Calm the patient
2...check for injuries
3...call security
4...restrain the patient
The answer will blwo your mind or get you more confused.
This is not a nclex question, it is something I somewhat made up based on a question I saw in a well know book.
yes it is hard, especially for us working in the field. i now know that it is a liability if you worked in the field prior to the nclex.
for example,
i had a quesion about being a nurse in the ER, and a comabtive pt was brought in. now we all know in those cases, in the real worl, we call security as stand by
i was security manager of kaiser hospital for 4 years before i was a police officer and ER standbys were a common thing
now, in the question, it was something like 1) call for securty standby, or 2) have an LVN stay with the pt
now, in the field from my experiences, nursing staff is always short--we cant afford for an LVN just to stay with a pt, not even a CNA for that matter-so we all call security-but the answer was to have the LVN stay with the pt---the rationale said-"remeber, nclex is not the real world-you have plenty of staff and time"--
i am re-training myself to think nclex world--once i pass i can "delete" all this and do the "right" thing.

I wonder if the OP is really verbatim. I remember questions like that..and when you really went back and reviewed..you'd see that they never mentioned oncomming traffic. Exp..maybe it said homeless man was on the street..but no specifics...THEN..when you saw the oncomming traffic question your mind goes in that direction. Remember..in NCLEX world...1 question is often there to mislead you.

The thing is whenever you have that kind of questions, try to picture the patient as he is. His current problem and analyze what really is the question asking.
Just remember the basic pricniple: what is the best thing to do to prevent patient harm? what will corerct the problem? how can we correct the problem?
I hope that you will also pass the next time around. As I said, sure, you will pass...Couple your efforts with prayers. Okay?
By the way, we're both male nurses...hehehe...sure we rock!

ABC is first but in ny mock scenario, his ABC were not compromised, (they will be in a little bit when a car hits him BUT thats READING into the question) remember, only go on the information contained in the queston
so with ABC's covered we do MASLOW
step 1-physiological needs-food, water, SEX, elimiation..
THEN safety.
heres one for ya , (another "mock" questions) that uses two MASLOW 1st step.
1. The nurse is caring for a 27 year old man who recently was found alive on an island alone after his boat crashed 3 years prior. He is malnourished, shows no signs of resp distress, and is alert and orientated. what would be the priority nursing action.
A-Call the doctor (i love this one)
B-get him paper and pen so he can begin his novel based on his expereinces
C-call his girlfriend for, a "special" visit, (its been 3 years)
D-Get him a big mac super size meal
:)
as you can see, according to maslow, physiological needs come first, food and sex are both included here. hmmm decisions, decisions
