Question about rocephin

So my question is... How many of you mix xylocaine with rocephin instead of sterile water?

My reasoning is... I have always been taught to do so. I worked in Peds for 4 years and all the peds nurses that I worked with and I always used xylocaine with our rocephins. Well now I work somewhere else and people keep using sterile water so when I asked why they said that they cant mix the xylocaine with rocephin on infants/toddlers... Has something changed that I am not aware of? One of the nurses even gave a gram of rocephin to an adult and used sterile water instead...

Any info would be greatly appreciated.

At our place (rural community health center) it was just PNP preference that we not give lido with rocephin.

We gave it w/ lido everybody 5ish and up.

Ouch. Guess that PNP has never received Rocephin IM without lido.

If I had a choice between lido for my IV start and lido for my Rocephin injection, I would go for the lido with the Rocephin.

The mix of Rocephin and lido has been done for at least 20 years around the world....If there was a problem with it, it would have been nixed many years ago. Even home Rocephin administration kits come with Lidocaine as a diluent as seen here.

And check out this article from the Archives of Pediatrics and Adolescent Medicine as far back as 1994. Definitely helps.

And here, here, here and here as well. Need more? Lidocaine is mixed with propofol and with penicillin G benzathine to help reduce the pain as well.

Unless the docs can give me documented evidence of problems, or unless the patient is a neonate or allergic, they are getting lido with the Rocephin if I am giving it.

Please be brave enough to ask the provider to explain why they choose not to use it, and it would be even better if you could post their documentation. I think we would all benefit. If they cannot tell you why, maybe your questions will stimulate a journal search for them. Patients deserve pain relief. Just ask JCAHO.

I don't work there anymore....sorry. I work mother/baby. I give lido w/ the mamas, none with the babes.

We only had sterile water to mix it with. There was one resident who got it often and she was not a happy camper when that happened. :no:

As I remeber it, the insert gave the mixing order as with lido or sterile water. With that said, I'm curious as well as to the MD specific order to use lidocaine.:twocents:

Must be part of the medical-pharmacy protocol (just like how Ambien gets substituted with Restoril unless specified/do-not-substitute otherwise).
