do you have a special form they must sign to release records?
would you want to share that form with me?

Nope. Why would a parent need a release?We have two criteria we check:1. That they are indeed the parent.
2. That the child is not 18.
We have release forms that cover parents authorizing release to OTHER people.

Parent can have them.
I release medical records directly to students/parents during the senior exit interview, which is right before graduation. I do this so students can take any info they need with them to college.
The only form they sign is they they received the records. Is that work you mean? Mine is simple, just staying what date I released the records and to who (parent/guardian/student over 18).

I ask for a request in writing, then I give it. No special form.

Depends on the school and the state....and the type of records they're requesting.
The school I worked in previously had a form and a procedure for immunization records and such.
If the parent wanted medical record data from our EMR (notes vitals etc) that was a big thing and usually the supervisors handled that. (needed subpoenas and such)
Current school will release physicals/immunization record back to parents as long as they are listed as the custodial parent. Not sure about medical records outside of that.

No special form, I just made a note that the parent requested a copy of medical documents since it was via a phone conversation.
Then the parent never picked it up after I made all the copies... but that's my life ?

No forms - parents are allowed to have a copy.

It's illegal under FERPA to deny the parents the information. Once we've verified we're talking to the parent (and it's not limited to the custodial one), we're not allowed to throw up more artificial walls around the record.
Thank you everyone. I don't have a problem at all sharing the records, but just didn't know if there was a reason I should have them sign they were receiving the records.
