Prewritten Photocopies of CII Forms

I live in MI. I am writing to inquire how to report a facility that is having their nurses utilize presigned photocopies of CII forms.

What does that even mean??

That means that a MD will sign one blank Controlled Substance prescription and the facility then makes photocopies and use them for several different patients

I'm surprised that the physician does this to begin with. After that, I'm surprised that any pharmacy would fill it.

Our pharmacy accepts faxed signed prescriptions from us, I assume that one does too, so they can't tell it is a photocopy.

I'm not sure how it is everywhere, but in Washington state I can fax in a controlled prescription but when the pharmacy brings me the medication I need to turn in the hard written original prescription.

Controlled substances have a limited amount of refills allowed (I think 6 on same prescription)... the MD would have to agree to that prescription once they get asked for a refill because it usually gets sent directly to them electronically.

How many refills on is that blank prescription? 0?

Also your user name says you are " determined to ban nurse"... you won't be banning only 1 nurse... the whole facility will get investigated and EVERYONE involved will get disciplinary action. Including the MD.

That was my original user name prior to becoming licensed and I never changed it.

My user name says determined to be a nurse where did you get an extra N from. It is always that one person that wants everything to be a conspiracy. I have told the nurses there not to use them and they still do. Some truly don't think they are doing anything wrong as they are being told to do this by DON and administrator. The pharmacy has warned them in the past to not utilize these copies and they continue. It is evident that they don't have to turn in anything they are all computerized hence how they get away with it. Thanks for your input.
