Can someone tell me where I could find a website to help with postpartum care plans? I have a careplan book but it doesn't have anything specifically for postpartum. My textbook also has very little information. Thanks in advance!

i'll keep an eye on this post. i need some for lady partsl birth, c-secion, postpartum and newborn. i have a few, problem is they need to be in order of priority. i can't seem to find anything online, and my book does not have them

I don't know of a specific website but I bought a nursing diagnosis book that has maternal stuff in it. I recently used it for my postpartum NP. It's called Sparks & Taylor's Nursing Diagnosis Reference Manual 6th edition. Also, a lot of nursing diagnosis not specific to potpartum are still applicable. Best of luck.

I have a nursing diagnosis handbook too- it has diagnoses divided by conditions in the front- its by Ackley and Ludwig. I would probably go with risk for ineffective breastfeeding, risk for infection (esp c-section), risk for exhaustion, risk for altered body image, anything related to pain, altered thought processes (post-partum depression), constipation (narcotics+ fear), ineffective coping, etc. You would be surprised by how many of the normal ones apply, esp for c-sections. My most important would be ones related to breastfeeding- my personal pet subject. Good luck.

ha ha... roll how funny... i needed exactly the same information few weeks ago when i was doing my care plan.. i did my care plan on risk for infection for post partum mom. i found maternal-nursing care plan book at the school library. if you are doing a care plan on this diagnosis, i can email you my care plan to you.

hi! how are u? It's funny how many nurses or student nurses need postpartum careplans.. i'm sure u've received many messages asking u to email them ur careplan.. i was wondering if u have time to email me some too.. or even priority nursing diagnosis.. i already did knowledge deficit r/t breastfeeding and altered pain r/t c-section.. but i cant do the same careplans twice.. i mean for my patient, I THINK the priority is altered pain r/t c-sec cuz this is her first c-sec our of 3 deliveries.. so im lost..

you are not going to find any postpartum care plans on the internet. sorry. care plans are pretty much custom made. you will find information about the writing of care plans on the nursing student forums and in particular on these two threads:

you will also get more help with a care plan if you post on the nursing student assistance forum or the general nursing student discussion forum. you can link to them by clicking on the "students" tab above.

all care plans, regardless of what type of patient is involved, begin with doing a thorough assessment of the patient. a nursing diagnosis is nothing more than a label that is descriptive of the problems the patient has. the patient's problems are determined by the things you find during your assessment that are not normal. those abnormal things which are really the patient's signs and symptoms (nanda calls them defining characteristics) become the focus for your goals and nursing interventions--not the nursing diagnosis. so, put your work on this care plan into your assessment of the patient. you will find assessment information on these threads:

you want to compare a postpartum assessment with what you found in your patient to see if you missed something and then correct yourself. this is how you will get better at assessing. from your patient's signs and symptoms you use a nursing diagnosis reference book to choose the correct nursing diagnoses. most of the time the diagnoses are prioritized by maslow's hierarchy of needs unless your instructors are telling you to use another system.

Post #8, the one right above yours, addresses your request very nicely.

This forum doesn't provide careplans for use as a guide; we assume you have a textbook that will do that very nicely for you.

I love care plans!! Ok not really but they do serve a purpose. I'm suppose to be writing one right now but Ive managed to get a little side tracked. Care plans are probably my worst subject in nursing school but I have to admit they have really helped me in understanding the process of patient care. Any way, just in case any one else stumbles across this post I do find that the text books we have are very handy in creating care plans (sometimes I quote them for interventions or rationals). A few suggestions for postpartum nursing diagnosis could be Impaired Skin Integrity (episiotomy, laceration), Deficient fluid volume (hemorrhage), Risk for infection, Ineffective coping.... oh there is so many! As my instructor always says... "What does your pt need?" I hear it alot.....:typing
