I've been a participant in the Nurses' Health Study 2 since I graduated. Once or twice a year, I fill out a questionnaire. These questionnaires are now online, and easy to fill out. Sometimes they ask for donations such as blood or hair. You are not obligated to participate in that if you don't want to do so.
