Hi everyone,
I am currently working on the application for the nurse corps repayment program 2015 and I need some help from someone who has also done this process before.
My question is in regards to the documents you have to upload:
1# Authorization to Release Information, Authorization to Release Employment Information- Do you print these off, sign, scan, and then upload to the site?
2# Curriculum Vitae/Resume- is this just a basic resume stating your current position you hold, and do you address it to the Nurse Corps??
3# Finally, proof of citizenship can I just scan my drivers license and then upload it to the application???
I would appreciate all insight please, I want to make sure I get this right the first time. Thanks ahead of time for looking I really do appreciate it.

and the grid at the bottom. Wow now I'm confused. Mine is also non profit non disproportionate share according to the app. I thought I was a tier one all along
Hi all, so I have yet to hear anything about my application yet, it still says under review. I am wondering if they are still awarding people or if at this point if you haven't heard, you would be just waiting for a decline? Has anyone heard of people being awarded this late in the process? I believe that from what I have heard that I am in tier 1, I work at a nursing home but the BHW customer service person told me that since my facilities HPSA score was 15 it put me in tier 1. Not sure really. I believe my DTI is around 120-150%. Congrats to all the finalists, I would just like to know one way or the other so I can breathe. By the way I am an RN/BSN
Has anyone else gotten the second signature/welcome packet? I got my finalist email on 7/28 and signed the contract right away. Nothing but crickets since then... I know it's a good place to be since I got the finalist email but I'm getting so anxious waiting for that second signature on the contract!
I thought that for hospitals it just went by HPSA score and non-profit status. I'm under the impression that the disproportionate share part is only for nursing school faculty, and refers to the socioeconomic status of the students at the school. The HPSA score tells you how underserved the community surrounding the hospital is, so by that logic if you work at a nonprofit hospital with a high HPSA score you're tier one, period. That's just my understanding though so I could be wrong!

I do not believe that the non-disproportionate makes it a tier four. When I did my research my hospital was a tier one and was the same non profit non disproportionate share. There was a website that gave you that info. I am going to look for it and post it for you
