Hi everyone,
I am currently working on the application for the nurse corps repayment program 2015 and I need some help from someone who has also done this process before.
My question is in regards to the documents you have to upload:
1# Authorization to Release Information, Authorization to Release Employment Information- Do you print these off, sign, scan, and then upload to the site?
2# Curriculum Vitae/Resume- is this just a basic resume stating your current position you hold, and do you address it to the Nurse Corps??
3# Finally, proof of citizenship can I just scan my drivers license and then upload it to the application???
I would appreciate all insight please, I want to make sure I get this right the first time. Thanks ahead of time for looking I really do appreciate it.

Received My finalist email this evening also:) so excited what a blessing. Debt to income above 200% probably close to 300% with a tier one I am a MSN RN who works in the ER as a staff RN. So excited this is Life changing for me and allows me the chance to live again without this burden on my back. Every nurse deserves loan forgiveness for all we do everyday. good luck everyone
For all those that have received your finalist award notification, did you just electronically sign the contract? Did you do anything else besides this and entering your bank info? Yesterday was an adrenaline rush kind of day at work followed by this exciting news. I want to make sure I did not miss anything. Thanks for the help!

I got the email on 7/28 stating that I was a finalist. Confirmed my place of employment, gave bank account info, and signed a contract! I'm wondering if anyone has ever made it to this step and not been awarded. Im anxious. More anxious now than what I was before I got to this step. I applied last year and got denied. My facility just moved to a part of town where no one has transportation to get places (we moved to obviously help our patients) but it bumped me into tier 1 and I have a student debt to income ratio of 287% :-/ I hope everyone else starts hearing more good news!

For all of you that are listing what tier your facility is classified as, where do you find this information? My facility has a HPSA score of 19, but no where on the page do I see what tier they classify as.
I am using this link to find my facility in which I input the address: HRSA - Find Shortage Areas: HPSA & MUA/P by Address - Version 2.0
My facility replied to my employment confirmation that they are a Critical Shortage Facility as well.
