Does anyone readmit newborns from home back into their NICU? If you do, can you please share your criteria/policies with me? Our neos feel NICU RNs better care for these infants than the ped floor. Wanted to know what everyone else does. My email is: [email protected] Thanks!

We regularly admit babies up to 48 weeks PMA. Older babies are accepted on a case by case basis.

Our NICU policy is that once a baby is discharged, either from newborn nursery or the NICU, readmission is to Peds.

If they get re-admitted for RSV, NO!!!
We did have a baby who was very complex, finally d/c'd w/home nursing, vent support, etc. Two days later, Mom called me @ home, asked me to come; long story, she got readmitted w/resp fail, but to peds ICU. Next morning, myself and Respiratory went up to check on her--they had a float taking care of her. Float didn't realize baby was almost agonal breathing. Went back to the unit, grabbed an attending, brought him up to see her. We all three wheeled her back to NICU. She spent a couple more months and went home again.
That girl graduated from high school a year ago.

It is a case by case scenario. If the baby went home within 72 hours, and we have beds available, they can come back to us. Otherwise they go to PICU or peds, but they do consult with our Neos since the kids are followed in our Preemie clinic by our attendings. We will admit older kids that need laser eye surgery and who need to stay overnight due to sedation but they go in our isolation room.

Board of Health forbids NICU readmit after D/C. Babies born out of hospital go into, and stay in, isolation for the duration.
We don't isolate the ones born at home unless there is a reason, like born in the toilet

We take ANY baby if they show up at our hospital, even if they weren't born there.
We also have an abundance of isolation rooms- I wonder if the policy would change if we didn't.

We isolate babies for 7 days if they had gone home or born at home.
