HELP ASAP!!!! I am actually looking for somebody who can help me with my application for Illinois Nurse Licensure. I am on the process of applying for NCLEX. I have registered on-line at CTS and presently gathering documents required. In line with that, cant find where I can get the fingerprinting card for illinois. Is this card to be sent together with my application forms or be sent to Illinois State Police with fee required? Is my CGFNS cert suffice for CES? Do they accept both IELTS or TOEFL? tnx! ;)

Try entering the International Nursing Forum. There's a thread there that discusses how to apply for an RN license in Illinois. It's more detailed and answers all your questions, but it will be redundant if I write it again here. I believe it was posted by Nxt777 and Kathykaye.

you send the completed fingerprint card and processing fee to the IL state police. I think the address is on the back of the card.

And you are applying for licensure, you cannot apply for the NCLEX. You have to be given permission to sit for that exam.

*DONT mail the card to illinois state police because they wont send you a reciept.. unless you go there personally. it happened to me that's why i sent another fingerprint card to an approved vendor (IDENTIX - http://www.identix.com)

*CTS needs the fingerprint reciept. it is a requirement...

Dear Kathykaye, I thank you for sooper informative threads. But there are still some things that confuse me. I hope you can still be of help.

I learned that I can get fingerprinting cards from US Embassy. I just went there this morning and got 2 cards. The ORI box was labeled with NM920190z NM BD NURSING ALBUQUERQUE, NM. I asked for Illinois fingerprinting card and the receptionist told me that these are the same forms they've been giving. Just curious if this kind of FPcard can be used for any state.

Will send request for FPcards from Illinois Dept of Prof Reg via email add that you posted, how long shall I wait for these PFcards? Any due payment for these cards?

Will it be Okay if I will go to NBI Q.C. instead NBI Carriedo?

So after I have my fingerprints on the card, will mail to IDENTIX. Please confirm if I got the right add: 1650 Wabash Suite D Springfiled IL 62704. Any due payment to be sent with the PFcard? Will I be receiving any confirmation that they received my PFcard? Or it will be receipt that I will be receiving? How long will it take?

The receipt will then be submitted to CTS. Shall I wait for the receipt prior submitting other requirements to CTS?

I requested my fingerprint card through e-mail, which was provided by kathykaye. The card came via air mail about a week after. I'm not sure if the card you got from the embassy here is the same one used for Illinois, but to be safe, just request the correct card from IDFPR.

Based on experience, processing your fingerprint card at NBI Carriedo is much better than doing it in QC. It's much more convenient at the Carriedo office. The QC branch is so crowded and you're made to wait and queue outside--under the scorching sun. Unlike in Carriedo, where the lines move fast and you're inside an airconditioned mall. Just go there early and avoid going on a Monday. At the entrance, buy a P20 ticket and proceed directly to room 604. After signing in their logbook, you'll be given another piece of paper which you'll present to the fingerprinting guy on the 5th floor. You're all set after that. Send your card to Integrated Biometric Technology (formerly Identix) 1650 Wabash Ave. Suite D., Spingfield IL. Oh, don't forget to enclose the $59-bankdraft and a note asking IBT to send a receipt to CTS once they're done processing your card.

the card is free.

call the fingerprint vendor (IBT) first and ask them bout the fingerprint processsing fee.. go to their website you'll find their contact number..it might be increased.. just to make sure..

you can go to any NBI office..

dont forget to enclose a note to forward the original fingerprint reciept to CTS.. coz it is a requirement..

*you can submit your application to CTS even without that.. they will wait for that.. they will send you a letter of approval if all the requirements are already there..

Hi KathyKaye (And to you NXT777 & Flight027 too!!! ) You wrote here that I can send my application to CTS even without the FPcard. What about the 1-page form CERTIFYING STATEMENT of FINGERPRINT SUBMISSION? Need I send this together with my 4-page application Forms even without actually sending the PFcards yet?

But guys, I don't have these PFcards upto now. I have requested via E-mail and even sent them request via air mail. Have tried calling them several times using the # that I got together with the packet that I have down loaded, but no body is answering.... of course I gave them calls during their office hours.

After reading all posts on questions re IL licensure application, I was enlightened with what I am suppose to do. But still, some questions are yet unanswered. Anyway, I am planning to send the requirements this week. Are the Forms that are available for downloading at idfpr web site exactly the same as the GREEN FORMS they've been referring to? If Yes, Kindly check what I am missing:

1. 4-page Application Review o.k.

2. Supporting Documents

a. Application fee $79 in bank draft o.k.

b. ED-NUR form n.a.

c. CGFNS or CES Report o.k.

What is Criminal Background Check Request? Where can I get it?

What assurance do I get if I will ask IDENTIX to mail the FP receipt to

CTS? Did you do the same? Was it a success? :nuke:

Shall I give them a copy of my IELTS exam? Or is this a big NO NO?

What CGFNS report shall I send? The original one? Ive read in 1 post that

I need to send them the original copy because they will be returning it

to me anyway. Need I submit also my Visa Screen Certificate?

yes you can submit your application to cts even without ur fp cards. they wil inform you of their receipt of your application and will tell you what documents you are still missing. they will wait for your application documents to be completed and will tell you if they have approved your application for licensure examination.

i sent my 4-page application, $79 bank draft and the 1-page certifying of fingerprint submission, i think thats the fp-nur. the fp-nur is just stating that you actually send your fp cards to one of the vendors. definitely, you'll do that wen u rcv ur fp card. even if you haven't send your fp cards to the vendors you can still send it together with your application forms. or you can send it separately when u have sent your fp card to the vendor. (which will cost you more in terms of mailing/shipping). tip: send your documents ( i mean all required documents) in timely fashion, anticipate the time of shipping so that cts may rcv them in short interval after another). doesn't matter, they will wait for the receipt from one of the vendors you actualy sent your fp card. when cts rcv that receipt, they will know you actually sent your fp card.

no need to call them. just wait patiently. it will come. may i know when did you email them or sent your snail mail?

criminal background check request - its actually the fp card you have to fill up. send to one of the vendors. and this vendor will be the one to forward the electronic fp to fbi and isp to check your criminal background. if you haven't been to the us obviously, your criminal background check wil not be a problem as to you dont have a file or record with them.

proof of fingerprint submission - thats the 1-page certifying of fingerprint submission plus the receipt from the vendor where you sent your fp card.

im not sure bout the receipt from identix coz i didnt go that route. as i've told you i asked my relative to send it to one of the vendors there in il, in chicago particularly. and the vendor issued them a receipt. and that receipt was sent to cts.

ielts - they dont need that. dont bother to send one to them. save your money from asking idp to send a score report to cts. in your case, ielts result should only be forwarded to cgfns/ichp for cgfns certificate and/or visa screen certificate.

cgfns report - the only report required by cts from cgfns is the ces. nothing more. if you will recall when you applied for the ces report, you have to enter the report recepient. and that report is downloaded by cts themselves directly from cgfns. you dont have to send them any report. cgfns will send you an applicant copy (your personal copy) of the ces report.

visa screen certificate - cts does not need this. cts is concerned only with licensure process and not with immigration process. vsc is need only for immigration process and you will present this during the consular interview in the us embassy.

i hope i've eased your confusions.
