What about the 1-page form CERTIFYING STATEMENT of FINGERPRINT SUBMISSION? Need I send this together with my 4-page application Forms even without actually sending the PFcards yet? But guys, I don't have these PFcards upto now. I have requested via E-mail and even sent them request via air mail. Have tried calling them several times using the # that I got together with the packet that I have down loaded, but no body is answering.... of course I gave them calls during their office hours. After reading all posts on questions re IL licensure application, I was enlightened with what I am suppose to do. But still, some questions are yet unanswered. Anyway, I am planning to send the requirements this week. Are the Forms that are available for downloading at idfpr web site exactly the same as the GREEN FORMS they've been referring to? If Yes, Kindly check what I am missing: 1. 4-page Application Review o.k. 2. Supporting Documents a. Application fee $79 in bank draft o.k. b. ED-NUR form n.a. c. CGFNS or CES Report o.k. d. CT-NUR Form (orig & current) o.k. c/o PRC e. CT-NUR form from states..... n.a. f. Verification Req from NURSYS n.a. g. VE Form n.a. h. Proof of Name Change n.a. i. Criminal Background Check Request ? j. Proof of Finger Print Submission ? k. TP-NUR Form n.a. l. Copies of Active Lic (temp pemit) n.a. m. RS Form n.a. n. Refresher Course n.a. What is Criminal Background Check Request? Where can I get it? What assurance do I get if I will ask IDENTIX to mail the FP receipt to CTS? Did you do the same? Was it a success? :nuke: Shall I give them a copy of my IELTS exam? Or is this a big NO NO? What CGFNS report shall I send? The original one? Ive read in 1 post that I need to send them the original copy because they will be returning it to me anyway. Need I submit also my Visa Screen Certificate? Please give replies SOON!!! Dear Kathykaye, I thank you for sooper informative threads. But there are still some things that confuse me. I hope you can still be of help. I learned that I can get fingerprinting cards from US Embassy. I just went there this morning and got 2 cards. The ORI box was labeled with NM920190z NM BD NURSING ALBUQUERQUE, NM. I asked for Illinois fingerprinting card and the receptionist told me that these are the same forms they've been giving. Just curious if this kind of FPcard can be used for any state. Will send request for FPcards from Illinois Dept of Prof Reg via email add that you posted, how long shall I wait for these PFcards? Any due payment for these cards? Will it be Okay if I will go to NBI Q.C. instead NBI Carriedo? So after I have my fingerprints on the card, will mail to IDENTIX. Please confirm if I got the right add: 1650 Wabash Suite D Springfiled IL 62704. Any due payment to be sent with the PFcard? Will I be receiving any confirmation that they received my PFcard? Or it will be receipt that I will be receiving? How long will it take? The receipt will then be submitted to CTS. Shall I wait for the receipt prior submitting other requirements to CTS? HELP ASAP!!!! I am actually looking for somebody who can help me with my application for Illinois Nurse Licensure. I am on the process of applying for NCLEX. I have registered on-line at CTS and presently gathering documents required. In line with that, cant find where I can get the fingerprinting card for illinois. Is this card to be sent together with my application forms or be sent to Illinois State Police with fee required? Is my CGFNS cert suffice for CES? Do they accept both IELTS or TOEFL? tnx!
