What resources are available for a homeless patient that is being discharged but he needs further care? He has been in the hospital for the max days allowed for his condition. How would you handle this?

Not for nursing to "handle."

Case management should have been consulted/following soon after admission. Aside from collaborating with the treatment team, you've done all you can do.
One problem with some of the homeless is the deliberate lack of interest in making responsible decisions.

Casse management or social worker should have been involved working on post dismissal things like this

I've never heard of anyone being discharged if they aren't ready, regardless of reaching a "maximum number of days"
I believe we are being asked to answer a homework question. That's why the experienced among us are not providing answers.

Aren't there any social workers at your hospital? I don't know if they're usually in hospitals or not, but my LTC/rehab facility has some who deal with these kinds of things - and sometimes quite well.

And your thoughts are? Is this a nursing responsibilities? Which referrals are within nursing scope?
Actually I am a BSN RN, who works in utilization review and am currently working on my MSN. I thought maybe I could get an idea of community resources in different parts of the country. I am sorry to have troubled the allnurses group.
Explaining this would have garnered very different responses. If you asked what community resources are utilized elsewhere..
