OK, I need pointers I am in my first semester nursing and we have evals on health assessment head to toe UGH! ? I am freaking I was very sick ? ? the morning we had lecture on this and am in need of some kind of checklist or video to show me more of what to do anyone out there have any hints for me please. Thank you all!

This is a great thread and there are some amazing links on here!! Daytonite lives on through her posts...students and nurses will continue to learn from her every time they read a thread she was part of! Anyway...I saw lots of threads for geriatrics and pediatrics and even school aged children, but what about adolescents? Does anyone know of a link for assessing teenagers? I have tried googling but I can't seem to find anything that stands out. What is done differently for assessing a teenager as opposed to middle age? I found one site that stated the only similar experience they have to compare an assessment to would be a school test and therefore they'd have anxiety and concern about giving 'right' answers. I'd appreciate it if someone could point me in the right direction!!

What is this alert and oriented x 4? I've been a RN for 3 years, and even in nursing school, we were always taught alert and oriented x 3, i never heard of x 4...now more recently i've been seeing professionals using alert and oriented x 4. I know alert and oriented x 3 is person, place, and time, but what is this 4??? I feel like an idiot for having to ask this, but honestly we weren't even taught a&o x 4 in nursing school, only a&o x 3. Thanks for the help.
