Good templates for Head to Toe Assessment

Just wondering if anyone has any good templates as a guide to do your head to toe assessment. My instructor gave me one but you have to write everything out and we don't exactly have time for that. I would like one where you could just go through and circle or check stuff off. If anyone has any templates they would like to share or any suggestions I would greatly appreciate it. Thanks!

Not sure what point you are in your program but I find it useful to use Gordon's functional health patterns, for our required clinical paperwork it gets all the data we need for the most part. It might not work if you are just learning though because it isn't listed in the order you would go, I just like it because it has many of the options listed...

But I just find that if I go in and try to remember the assessment I tend to forget things, so I'm trying to find a quick, easy way to do an assessment which doesn't leave me walking back in the patient room because I forgot something.

I like that pdf because it has lots of descriptive words, and I am often at a loss for how to describe things.
