Does anyone have a consent form they use/like for a hearing evaluation? At our school, a speech/language pathologist comes to do the screenings so I do not have a general consent form.

Since screenings are mandated by the state, we don't ask for consent.

^^Ditto. Unless I have some reason not to (under MD care and specifically ordered not to) everyone gets grade appropriate screening.
I know at my own kids' schools, I still have to sign a consent form for the state mandated screenings (BMI, Hearing vision, etc.). I work in a different district (same state) and am at a charter school.

My state of MA is an opt-out state, not an opt in state when it comes to screenings. I notify parents of screenings and let them know they can opt out of hearing and vision in writing, but I do not need an opt in.

In Illinois we need to inform parents that screenings will be done. It is in our handbook and I believe it goes out in the newsletter.

I had to send out a notification this year that we're going to be doing the screenings but it's usually in the parent handbook.

No consents here. I do send a "heads-up" email to the parents letting them know I will be doing them. Some will then produce paperwork if their child has a current diagnosis, which I then don't screen that student. I've yet had a parent refuse it though.

No consents for my district. It is in our handbook and parents electronically sign a form yearly that they have read the handbook. I do send out a blurb in our first newsletter of the year to let parents know I am starting them and that they will be notified of failing results. I do not screen those with documented hearing losses and are wearing hearing aids, cochlear implants etc or those students with documented vision issues outside of the usual near/farsightedness.
