The Association of Healthcare Journalists has launched a new site where you can search violation reports by Hospital/state (limited to those that have been discovered through Freedom of Information Act requests). I'm sure there are some juicy ones buried in there but it's difficult to just browse them based on the type of violation.

Be sure to share if you find any really good ones.

I haven't found anything near that good, mostly failures to properly document restraints on intubated patients, pretty tame.

Pimple-popper-CRNA is definitely the front runner so far.

didn't read all of it, but what can be done to stop a miscarriage at that point in gestation? It is mentioned several times "no documentation of efforts to maintain preg." or some such wording.......
