Per the request of janina08 I've started the April NCLEX takers support group. When do you take it? What are you using to study? Would anyone like to review any of the systems/meds that he/she feels weakest on? Let's keep each other in our prayers and good thoughts as we go through one of the biggest experiences of our lives. :)
CONGRATULATIONS to EVERYONE WHO PASSED!Help me analyze this question from Kaplan please:
A client has a bovine graft inserted into the left arm for hemodialysis. During the immediate post op period, which of the ff actions if performed by the nurse is best?
1 restart the IV above the level of the graft
2 take BP on the right arm
3.elevate arm above level of the heart
4. check radial pulse of the left arm q4
answer is number 2. to prevent complications. how come? I still cannot grasp the rationale. Is it because taking BP at the left arm will cause complication so take it at the right? *sighs*
yes, we cannot take bp, put IV or any procedure to the affected extremity.That is why we need to take the bp on the unaffected arm.

When you have a dialysis pt, you have to be very careful with that graft. It is their only means of eliminating the wastes that are swimming in the body that their kidneys cannot eliminate. Immediately post op (again, think ABCs) you want to know how that pt's vital signs (especially blood pressure) are adjusting to that foreign entity in their body. You would never start an IV in the affected extremity because you run the risk of puncturing the graft and the vessels that the graft is using. I'm not sure why you would elevate it, and you would check the pulse (though, I'm not sure if it's q4 hrs), but even then, it wouldn't be the IMMEDIATE action taken by the nurse. You need to make sure that the pt is stable and that there aren't any complications due to the implementation of the graft. Hope that helped, and correct me if I'm wrong. :) (I'm still learing myself. )
hey, guys!! i posted my last message last april 22(philippine time) before i went to manila for my exam.. i had to stay there the night before the exam since our house is quite far from the testing center.. on the night before my exam, i cant sleep!! and there i was crying in the bed.. my boyfriend who is with me cant sleep either.. and he's trying his best to make me laugh but i cant.. it's past 12 midnight(my exam will be 9am), i left the house and did some walking..hahaha.. while im holding my ipod and hearing the song, "lead me lord".. after that, i came back to go to sleep, i wore a rosary and the next thing that i knew, my alarm was already ringing(it's 5 am, u see i didnt get enough sleep)..
okay before i got out of the bed, i said a prayer.. i thanked the lord for the beautiful day, i know im already blessed for all the things that i have. but i will be more blessed if he'll enlighten my mind before my exam, then i asked him to guide me, give me strength, courage and determination.. and asked the holy spirit to guide me..
i reached the testing center at 7 am, so i was too early for my 9am exam.. so my boyfriend and i just went to mcdonald's near the testing center to have my breakfast, and there i was yawning a lot! omg!! my boyfriend is telling me to drink coffee.. i told him i dont want because i might get nervous..hehe
at 8:30, we went to the testing center, and i was shocked because the examinees are already lined up. so i hurriedly went there.. ehehe.. and yeah, im at the end of the line so last to be admitted also.. i patiently wait while relaxing myself..
and when i entered the room,i prayed again..
and start answering my exam..
if you'll practice answering questions everyday, then when you take the actual nclex,you'll feel that you're just answering one of those practice tests..

and d rest was almost prioritization
finish the test in 2 hours
after the screen turns blue, i had these goose bumps:uhoh21:.. omg! it's finished. what have i done?? and the thing is i cant recall the questions anymore..
and so after the exam, we just went to eat and we got back to our house for me to sleep because i feel so sleepy.. at 6pm, i woke up and there, i can now remember all the questions.. as i just found myself crying.. i find some of the questions easy, so im thinking, is it easy because im not doing good in previous questions so the computer is giving me easy questions?!!
this morning, i left manila to go back to our province where i left my laptop,
upon arriving here, i checked the nm bon's website, and there i find my name with my license number!!!!!!!! oh my god!! thanks for all the blessings!!:bow:
