hi to allnurses,it's my first time to post a thread in here but i have been a member for quite sometime. It is such a good help reading forums in here..thank you!

Yesterday i received a letter from the BVNPT after more than 2 months waiting about the Declaration of Nursing Education and or Licensing.The questions here are confusing. Can someone here who got the same letter if you can help me fill it up please?..:(

did she say anything bout the number of hours worked and the date started and completed? like what the others do, i did not included mine cause i dont have a copy of my clinical rotations.

@proudpinay,.,yes i was able to complete all their requirements needed for the application, i submitted the sealed envelope that has tor, record of nursing program, and rle in it,...and i was surprised that they state in the letter that the board is requiring me to have them my sealed record of nursing program as to continue my application process,..i was thinking then probably that they just overlooked on my papers, and they also state that they had received my transcript from registered nursing board,..but they need my record of nursing program to complete the process as well as the declaration of nursing educ...anyway i should call them tomorrow for any clarifications on why they didn't have my record of nrsng program, and to ask them about the declaration of nursing, i still don't know what to answer though,...thanks proudpinay, ill keep u posted on what info i may get tomorrow..godbless to all..:)

hey mjhay. the same exact thing happened to me. have you got everything straightened out? did you finally get your att?

hi bluecross, how are u?..did you send your record of nursing program to them and they lost it too? that happened to me also,

they denied that they have it...so, what i did, i processed again from our school, good thing i have my mom their..and the school was very cooperative, it was done like a day and my mom send it to me afterwards....and then after 2weeks i received my record of nursing program, and mailed it to the BVN immediately along with my declaration form..after a week i phoned the BVN evaluator and ask her if they received my record of nursing program and declaration form(just to make sure that they have it now this time), and she told me that they have it already, and i should wait like 6-8weeks for their response..so, it was a month ago now,and i have 1 more month to wait...im just hoping then that everything will be fine, and i would receive my ATT this time.....

once ur BSN grad..ul be qualified. i was on mu 3rd year when i applied for LVN yet approved.

that depends on how ur skul will fill up the form

what is the number of the BVN to contact them? we've been trying to reach their number but no one was answering

i called BVNPT last week to ask if they already got my documents that i send last month , and they already received it. they told me to call them 4-6 weeks from now because they still evaluating some applicants from june 28 , 2012 , so i asked them regarding DECLARATION OF NURSING EDUCATION.. they told me that they will send it to me after they evaluated my application, and i need to wait 4-6 weeks again, i asked them if they can mail it to me even though they are not yet evaluating my documents, for me not to wait for another 4-6 weeks . and the answer? is YES... so when i got the form from them i filled up the necessary information that they needed, luckily i have with me the copy of my CLINICAL ROTATION so i am able to put all the info's regarding my CLINICALS .. hopefully i will go smooth..

we have 3 days of duty in a week so its is equivalent into 24 hours

plus the number of weeks in a month 24 x 4 = 96 hours in one month if your rotation last for just a month... but it depends

96 hours? guys, what did you put there on number of hours worked? other answers?
