Please realize the following.

1. If the order says q 4 hours prn - even if you did not get pain pill #2 until 5 hours after pain pill #1, I cannot give you pill #3 one hour "early." You state pain as 0/10 on every reassessment, so this is not inadequate meds.

2. This is not a personal vendetta against you by me. Please stop ********. Please stop arguing.

3. I am acquiring one hell of a guilt complex and thus dreading my shift tomorrow.

Do what I do: I tell people straight now that if I give them meds before they are due (a) it can seriously damage their kidneys/liver, etc and some patients have even had reactions/died due to being overdosed from medications, (b) I am breaking numerous federal laws re giving medications © BUT I will get a Dr to reassess their pain. This usually works, people only complain because they have awful pain, and some people can't handle it pain at all. I also find giving them more TLC usually helps (but not always!). Some people that have pain or demand meds either have anger issues, or just don't like hospitals/nurses, whatever. They take it out on you cos you are just there at the time unfortunately. If they keep harrassing you get someone more senior to talk to them - that may help too.

I beg your pardon. I took the fact that the OP said "Patients" (plural) "Please realize the following" that maybe he/she had issues with most or all patients; in which case a career change may be in order.

"Go jump in a lake." If you wrote that as a nursing order wouldn't you need to specify a time frame or PRN? Don't forget to reassess afterwards for the desired therapeutic effect. I've never used that one, however I've often suggested a retroactive abortion for some patients. Perhaps both nursing orders have the same outcome, your order just being more tactful. I'm glad you posted here, it'd be a shame to let such feelings eat at you. Sometimes the hardest part of the job is to forgive the patient. Best wishes and hang in there sister.

Oh, no, not all. Not even close. It's just a perfect storm of a long shift, heavy patient load, and one person calling 3 times in the half hour before their meds were due to inform me that their meds were "late."

Everyone is aloud to have a bad day, without having to change careers- or I think that I would have to give my kids up for adoption : )

I think if you didn't get annoyed by these types of pts you would be abnormal!
