I believe #6 is referred to as a "Mental Health Day".
Back in 1972, THREE times I woke up, jumped out of bed, and immediately fell over as my right side refused to function. I had such severe dizziness I had to crawl along the wall to the phone to call out sick. I could not stop the spinning to my right nor stand up nor crawl back to bed, so I stayed on the floor until the episode stopped, which was a very gradual process.
Naturally, nobody at worked 'believed' me.
After the 3rd time it happened I saw a doc, who found nothing wrong with me.
Years later, my mother had the same thing happen to her and had a PRN med to deal with it.
We had a CNA once make a call on her personal cell phone while in a resident's room (witnessed by another CNA) to someone telling them to call the facility.... five minutes later after answering this call she runs screaming to the front of the building that her daughter has fallen down the stairs. Not the first time she has pulled something like this, and they still let her go home.
