Y'all.
I just legit wrote a note to the Tooth Fairy.
First grader swallowed the errant tooth (or maybe he lost it in the lunch room, who can say?) I called mom just to let her know. Mom said that two teeth were loose (I only saw one socket). Kid was worried because all his friends said Tooth Fairy doesn't come without a tooth....
And I wrote a note to the Tooth Fairy so the kid wouldn't be sad. Nobody warned me pre-K- first are so darn cute.

I said "Oh, bless your heart, let me get you in touch with someone that can help you. You need to say I am an old student in my city's ISD and I need a copy of my shot record, however, I am 42. Let me know if they were able to help you!"
In Illinois I believe its 50 years that we must keep all health records, so they would be in luck here! When I was in nursing school I was happy I went to the same pediatrician from birth until I was 18 because I just called them up for my records. I still had to have titers drawn though so I'm not sure why they bothered with the shot records.

I actually have saved a copy of Iz records form every graduate since 1995....have bailed out many a student going to grad school
