A neighbor of mine asked me last night about an incident at her fathers assisted living (not LTC) being reported to "state" - I thought she meant the Ombudsman but she said "no" this is a different agency. I have no idea what she is talking about as this facility is not JACHO certified like some so...who is "the state" that offenses in ALF are reported to?

I thought the state did SNFs and ALFs??
Who - what agency - for the state? That's what I don't know...she said the ombudsman is NOT the reportable agency - it's actually a business that is a step before (?) "the state" steps in - is the agency agency the Office of Inspector General? Department of Health? That's what I'm wondering - who does the inspections of LTC and ALF and gives out the tags etc?

If it's a long term care facility, the "state" is the Department of Public Health. Sometimes the DPH will call in the Attorney General depending on the nature of the issue.

In MN we have OHFC... Office of Health Facility Complaint.... it is only for LTC. For AL, it would be CEP... Common Entry Point, Adult Protection or DHS.

Office of the Attorney General, or Department of Social Services- Adult Protective Services, or the Office of the Inspector General. Ususally ALF are "surveyed" by the Department of Social Services.

It depends on where you live. In Texas, it is DADs. Most of the time you can contact APS to initiate a referral and will actually need to in order to get an intake number that you use as part of the reporting procedure. If you are still unsure call another local LTCF and ask them or talk to the Ombudsman. He/ she will have the correct information.

In Indiana it is Indiana depart of health for both LTC and ALF. I'm told we are also required to notify ombudsman as well.

I believe that the facility by law must post who and how to call if theres a complaint or pt neglect or any serious problem. Dept. of Inspections and Appeals is who regulates our ltcf. Look it up online for your current state that the acf is in. good luck.

In Oregon, "the State" is the Senior and Disabled Services division of the Department of Human Services. We are surveyed annually or biennially, depending on the facility type, by the Client Care Monitoring Unit which is another branch of DHS.
