Skin tag on assessment

Can anyone tell me if a single tiny skin tag is mandatory on a skin assessment sheet?

I would say yes...it is there and not "normal".

Actually, skin tags are general normal variants. Pretty much everyone gets them after a certain age, especially in folds (like axillae and neck). I wouldn't bother to mention just one unless it were being traumatized by clothing, etc., but if there were a flock of them I would. Big flocks of them can be associated with skin viruses and other stuff.

Well my dermatologist doesn't care to take note of mine so I'd say no.
