Yes. I have it as a checked box on my health form. On the asthma form I have I also list it again and state that if it is determined that student cannot use inhaler properly with supervision, that nurse will contact parent to determine next steps.
(FYI: for me this is goal for student to use inhaler properly on their own vs no self carry. But I work with older kids in grades 7-12 and goal is for them to work towards independence with their health care when they go off to college and/or into the real world.)
We require doctor's orders for any student to self-carry Epi or an inhaler, and I do strongly encourage all of them to self-carry. I am at a 9th-grade campus, and a lot of our students ride shuttle buses back and forth during the day between the main campus and ours. They need to have access to their medication at all times.
Yep, signed by doctor and parent. Not really any different than the regular (in my office) med authorizations but on a different form.
I fought hard years ago to allow my son to carry his glucometer/insulin/glucose pills with him. We had just broken the "inhaler" ice for self carry at the time.
