I need your help here!

Our department is going to change our current resus room chart. I am one of the members who will assist with the change. I would like to see your resus room charts in your departments to get a feeling about how you document resus nursing care. Will anyone be very kind to send me a copy of your resus room charts?

I am guessing resus room is short for resuscitation room?

ACLS flow charts are what we use.

OP, do you mean that you have different documentation / forms for critical patients than you use to document on other, less acute patients? And that you are changing this documentation and looking for input?

We use trauma flow sheets for or level 1and 2 traumas and obviously

code sheets for our cardiac arrests . The trauma flow sheet pretty much documents everything done as well as our assessment on a carbon copy paper. We also have conscious sedation flow sheets

Sorry for the confusion!!

I asked for "the assessment & observation sheet that you are using to document patient assessment details for all patients who are in your resuscitation room".

We are currently using code form for patients who are arrested, such as cardiac arrest. We also have a standard nursing observation sheet for all other patients in our ED. I am looking for a new nursing assessment and observation sheet for critically ill patient who doesn't require code form yet and is a little bit too much for a standard nursing observation sheet.
