Questioning Doctor's Order

Wanted to share this article from NSO and pose a question: A portion of this article states that we should document our objections and the response of the people contacted related to questioning an order that a physician chooses not to change. How would I do this? I have not come across this just yet, but if I did, how would I objectively document in a chart that an order was questioned?

You are correct. What could have happened to me is out of being so frustrated with the entire ordeal, I would be tempted to write ":banghead:Idiot physician who got his license from a matchbox told me to give such and such...I then said to myself 'he must be off of his rocker' and told him to kiss where I twist"... (exaggerating, but you get my drift...:lol2:)

Can you imagine?? "Nursing Note 11/1/08 2:20am: Reviewed and acknowledged that this insane order could not have been written by an educated individual. Collaborated with pharmacist, nsg. supervisor and DON for a good laugh. For good measure, informed patient of the insanity of following this order because he would be dead, nurse unemployed r/t the stupidity of following it. For the best interests of collagues, copies of above order copied, emailed and plastered all over the facility for all to witness in awe. Patient's name removed due to HIPPA, but no one said to protect the doctor. The world must know" Signed :nurse::down:

charting and observation !!!

sometimes you have a lot of resistant ab and one which may or may not have caused a reaction.

either the patient is not sure which med was taken

reaction could have come from another med taken at same time

sx of reaction could have been confused with sx of the disease which pt was being treated for

because a mild reaction can be followed by a life threatening one i would chart, alert cna to be an extra pair of eyes in observation and check frequently
