just for clarification, how do you write this... "take every 3 days" on the mar?
i know that qid is 4 times a day and qod is every other day.. is "take every 3 days" written as "q3days"? or "q 3 days?"

you should follow your facility protocol. healthcare organizations are moving away from the use of many abbreviations, due to concerns for patient safety. there are many that jcaho lists as "do not use." i would simply write it out in its entirety, "...every three days."

At my facility that would be written "q72h". It follows the "q6h" for four times a day, "q12h" for twice a day, etc.

And that is how med errors are made.
Read twice, administer once.

Ever since my employer started to stress the "Do Not Use" list in 2004, I have written everything out in plain English on the MAR. That way, you will be able to read that it is three times a day or every third day, and keep from making a med error.

So what was the question?

The original question was how to note on the MAR to give something once every three days, not three times each day. (Your earlier response would be nine times the intended dose.)
lol nice
Looks like I should read the forums instead of skim.

If you are just reading through then certainly skim, but seriously, if you are going to answer someone's question you need to read what the actual question is. If you post a question, would you want the answers coming back by people who just skimmed your question and gave you wrong information to use? Something to think about. A med error could have been made had yours been the only answer and OP took it as correct.
I just skimmed yours but could tell you're trying to bust me out. Thanks!
