i need nursing diagnosis for my patient she is 60 yrs and she do cholecystectomy and umbilical hernia since 3 yrs and since month ago she diagnosis with bile duct obstruction and now she is waiting for surgical operation. and she admitted form ER

Uh oh, not again. Am I the only one who wishes that there was a way to trace these NDx questions so we could give some feedback to the 'guilty' schools??? They're obviously not providing students with a clear understanding of how NDx are established.

Eman, You cannot identify Nursing Dx on Medical Dx. Nurses assess & treat human responses (physical & psychological/emotional; actual & potential) to disease..... not the disease itself (see ANA definition of nursing). First, you have to assess the patient to see how she is responding to her illness - then, compare your assessment findings/data to the defining characteristics of NDx to see which ones to choose.

If we hand you a nursing dx, you won't learn how to do it.

What are they presenting with? This will be the basis of your ndx. Are they in pain? Are they septic? Anxious? If they are septic then u have a number of priorities to look at and u dont put anxiety or knowlege deficit at the top.

People post all the time with an acute situation and propose the most random, plucked from no where ndx. To pick an ndx, u have to look at (or in academic cases, know about) the patient!
