Hi guys, who will fill out and sign the form to accompany transcript of training? Registrar or dean?

the dean filled out and signed my accompany transcript of training, though the additional forms they needed like the TOR, and breakdown of hours will be coming from the registrar

Upon entry in PRC Morayta, the first counter on your left is where you should go. That's the information desk. They'll be asking you what you will be processing, then they'll be handing over a form with written instructions as well as a list of requirements needed for your transaction. From there, the other steps will be easy. The part that took time the most (in my experience) was paying the fees at the cashier, since the queue was too long during that time.

Brief guide (please feel free to correct any wrong or misleading info guys):

- Get a form from the Help Desk/ Information Desk

- You'll be instructed to go to a certain window just in front of the help desk (Window M?)

- You'll be instructed to go the Legal Department located at the 2nd floor Main building (if I'm not mistaken) for some checking

- Just make sure you have a photocopy of your pertinent PRC documents just in case they ask for one (can't remember if they asked for it) like your license and board certificate

- Pay the fees at the cashier (around 200 Php, can't rememberrrr gahhhhd my memory cells!)

- Go back to the window in front of the Help Desk (Window M?)

- They'll ask if you want them to send it for you or if you would want to avail courier services. I suggest you avail courier services so you can track the whereabouts of your papers, and of course to have some sort of peace of mind that PRC is not the one tasked to send it on your behalf. (haha!)

- Courier fees are around 2300 to 2500 Php from FedEx for papers bound to UK. Can't remember the exact fee that I paid, but it's around that range.

Just an additional info...after all the processing you have done for the PRC VoL, you have the choice to send it with PRC themselves or with any courier service.

There is DHL located in front of PRC, you can just go there and present your receipt then they will be the one to pick it up for you and send to NMC, so you don't have to go back to PRC to collect the form then go to a courier service and send it.

After 1 week or when your papers are ready, DHL will send you the tracking number so you can locate your docs

After all NMC wants the licensing body to send the forms directly to them

I have done this since I am working overseas and i can't come back to PRC and send the form myself and it worked, i informed my agency batchmates and they have all done it. It worked perfectly fine...all the forms were received by NMC in correct time.

Oh by the way...the price is 2300k

Hey Malakite

I highly advise that you go to the PRC office and give them the form to be accomplished first. It usually takes about 2 weeks or so for them to complete the task. While doing so, you may also proceed with the processing of other documents eg those for your university, your work institution and your GP to fill out. Everything may be done at the same day, but in your convenience of course.

i saw from a friend's file that the forms that will be sent by nmc after the cbt exam has barcodes in them. the barcodes are specific for a candidate or is it just a barcode for the forms? because im thinking of asking her for copies of such forms so i can process them now instead of waiting after the cbt exam. thanks.
