NANDA Help Please. NCP Assistance as well.

Hello! I'm currently doing my second NCP in my Nursing life. I really don't have any idea on how to do it up until now. So please help me. I'm having problems finding the diagnosis of my patient. I need 2 actual problems now. Btw, this is due this afternoon. So please, I would really appreciate it if you'll lend a hand.

Here's the scenario:

My patient is having problems defecating because she has an episiotomy. She's a primigravida and she's 18y/o. It has been 4 days since she had delivered but she is stil not passed stools, only flatulence. She also complained of 7/10 pain in the episiotomy site.

What could be the appropriate and NANDA approved diagnosis for this one? Thank you very much!

Try doing it yourself first, then bringing your work here for feedback. People here aren't interested in doing your homework for you, especially if you let it go until the last minute.

I agree with Coffee Nurse. However, a suggestion---there should be a list of NANDA approved diagnoses in your text or somewhere that is appropriate for you to use UNLESS to use would be in violation of your school's policies and considered cheating. Have you asked your instructor to explain futher how to write a nursing diagnosis?

I did my own NANDA homework back in the dark ages, and fully expect you to do yours, but I'm willing to help you refine your thought processes if need be.

Second NCP??? Are you still in school?? You should have youre NANDA always with you.. I agree with coffee nurse you should do it first then post it here, if we will tell you the diagnosis you will not learn just from copying...

When I was still in school our CI always asked us to bring our NANDA's together with our drug hand book...

Nope, my professor din't explained the process further. We have to discover it all by ourselves.

(2) I still don't know the NANDA diagnosis for this one but since she doesn't want to ambulate, a risk for (?)

this one is ok.. but the first one is incorrect bec. you're patient is already constipated so NOT RISK.. ok

Remember that diagnoses of actual problems require subjective/objective data (signs and symptoms) to back them up. They should take the form:

[problem] related to [cause] as evidenced by [data: e.g. inability to defecate x 4 days, etc.]

Ok, if your professor hasn't explained further, it is really your responsibility as the learner to ask. Think about things, when you write nursing diagnoses, that are "nures fixable"==ie===those things you can do to fix the problem.
