I'll start my timeline here.

Background information. I'm a Philippine graduate, 2013, passed PNLE 1st try right after, and NCLEX-RN Texas 1st try on December 2, 2014. Didn't bother attempting to apply for CA-BRN, because I'd throw money at the concurrency, knowing that the 3 year clock would start. US born citizen with SS as well.

December 4, 2014 sent the following packet. (Also used this as part of my cover sheet for LVN application) (I edited out personal information) (Feel free to use)

*Payment of $150 in the form of a check to BVNPT

*Application for vocational nurse licensure in California, completed and signed

*Record of conviction, completed and signed. (2 pages)

*Sealed envelope from my college, containing my transcripts and record of nursing program

*Envelope with one 2x2 picture

*Finished live scan fingerprinting form

*Photocopy of my diploma from High School

*Photocopy of my Philippine nursing license

*Photocopy of my college diploma

December 6 sent email to BVNPT asking about the declaration form. I read up on AllNurses that some people managed to get the declaration form ahead of time, instead of having to wait for them. Basic paraphrase, some edits.

I am aware that it's too early for us to inquire if you have received it yet, but my concern is about the "declaration of nursing education" also known as the yellow form. I read online that we are able to request this yellow form ahead of time (Before or after sending our application), to help speed up the process, by sending it back to the BVNPT, before you guys request it from us.

So the question is, am I able to request two of these yellow forms to fill out the declaration of nursing education? If yes, may you send it to the following address?

Hello Good Morning,

We will mail you two forms, today.

Their reply was so simple lol. I should have asked them for the declaration form, before sending my application, so that it would all be in the same folder, but wasn't aware that we could get it ahead of time. I highly recommend that you get it before you apply, so that it's all together when they receive it. (They should really just include the yellow declaration form as a downloadable on their site)

So that's pretty much where I'm at. Now gotta wait 2 months or so.

Prolly, you waiting game is aproximately 6 months you already have your eligibility and ATT. it depends how fast you respond to your requirements :)

We'll just see. I'll update here on how it goes. I'm expecting 2-3 months. I should be getting the declaration form tomorrow or Thursday, which I'll fill out and send by the end of the week.

A college from Bacolod City. I saw in your posts that you are from FEU, I've read that people from FEU have trouble for applying to the BVNPT-CA because FEU registrar is having a problem with filling out the hours for the record of nursing program part of the application.

I also e-mailed them last night about the Declaration of Nursing Education form. I got a message this morning, "We will mail you the form today." Their response is awesome! So basically, I will send my application forms, fees, sealed envelope and everything all together because I will fill up the yellow form as soon as I receive it. I hope this makes the application process faster.

Yeah, it should be faster, because there's no down time for when they send you the declaration form and you respond to it. I have to deal with it now, with following up with them saying I sent it ahead of time. Hope this guide helped you out.

I also got my yellow declaration form today. Here's a few emails I had with them a few days ago, while I was trying to figure out how I would fill out the declaration form.

They said.

On the declaration form, you ask for two hospitals and hours worked. Thing is, my Philippine education throughout the 4 years had at least more than 6 hospitals all together. All of my hours are also broken up, for example, 8 hours for 3 days, every other week for clinical rotation. This went on throughout maybe 3+ years.

So my question is, how exactly do I fill out the portion of question number 5, that asks for Date Started, Date Completed? (Since it was throughout 3 years)

Also, how would I fill out the number of hours worked, if I was only in that rotation for 8 hours a day?

Would I also have to fill out the information for ALL of my hospital rotation? (Again, 6+ hospitals throughout the course of 3+ years)

Is it possible that we just give two of the main hospitals where we spent most of our clinical rotation, and just give a range of the hours worked all together? (It would be easier to find their director, address, telephone number, which I currently do not have) Would the following example suffice?

Yes you will need to get all that information and put in on the declaration form. If you have more than 2 hospitals please write the other hospitals on a separate piece of paper and attach it to the form itself.

Okay. Thanks for replying, i'll get the hospitals information, but you havent answered the question on whether i can lump sum the hours together from the years as shown in my example.

They said.

It is all an estimate. We do understand that a lot of schools in the Philippines, all float to different hospital for clinical. So, estimate the hours at each hospital and the date you were there. Your school is able to help and give you that information.

I said, to clarify.

So to clarify, our hours dont have to be exact, we can just give an estimate for the total hours?

They said.

Yes, that is acceptable because the board will call the institutions to clarify when you submit the form.

I will go with the lump sum method, but I'll list the dates together per hospital. Like if I had one hospital where I had 10 rotations, then I would write down the 10 rotation dates below it, like this.

I do plan to print it out like that filled out, and staple it to the declaration form, saying "Refer to attached paper".
