Medicare admittable diagnosis for SNF

Hi, I am wondering if any one can tell me some admittable reasons for some one to get directly admitted to a SNF? I have a relative that is declining, will be starting on hospice soon, and needs to be cared for 24hrs til her death. We are not able to do it or afford help any longer. She lives in her own home that is NOT paid for. She does not qualify for MediCAL. Her prognosis is terminal within 90 days appox. Medicare rules make it dificult and I don't know all of reasons. Only one I can think of is multiple falls in a certain time period.

Please help if you can, we need to know asap.

What about researching an inpatient hospice facility? If you are working with a hospice agency, perhaps they can recommend some local facilities and help to facilitate the admission & transition.

it may be that the snf doesn't want to admit her because they get paid less per diem for a hospice patient than on straight medicare. suggest her primary care provider admit her to hospital for an evaluation without mentioning hospice. if she is that unwell they should be able to get her admitted to a snf for continuing care, and then you get her admitted to hospice.

Medicare's hospice benefit can be somewhat difficult to navigate. I think if you contact your local hospice with your questions you will be able to get your answers.

In order for a SNF inpatient stay to be covered under Medicare, needs to have a 3 day hospital stay.

good to know. seems reasonable, so they would have complete info on medical condition and nursing assessments to see if the pt meets criteria for hospice and specify what materials/meds/etc would be paid for under the hospice diagnosis..
