As the crusty old bat society has told me many times on AN, nursing diagnoses are based on your patient assessment, not the medical dx. Took me two semesters to make that connection. And in the long run, made things soooo much easier because I could see it with my own eyes instead of trying to invent a careplan from a medical diagnosis that may or may not even apply to my patient.

Is giving someone the answers really going to help them in the long run? No, it's only going to encourage them to come back here for their next care plan instead of learning how to think critically about their patient's assessment and have nursing diagnoses specific to that patient. This is why we ask what students have so far and what they need clarification of/help with. Spoonfeeding is not the answer.
