International Verification of Diploma

I reapply to retake NCLEX RN California and asking me for verification of my diploma coming from my school in the Philippines and must verified from department of foreign affair. I did all the requirements and send it back to California BON but I got another letter again asking same documents. Anyone has same issue like me please leave comments what I have to do. Thank you.

I’m from Davao and all my documents from there too… after my relatives received the form from school to ched for CAV then they forwarded it to DFA for apostille , from DFA Davao should be sealed and my family member send it tru dhl Davao.. hope this helps. Good luck

Hi Rosey, did you include the international verification form in the mail? Did dfa signed and fill up the form? That’s my worries if its need to be included or not and do I need to send it to manila Dfa just for them to signed the form. All documents are ready for mailing except the form…Thanks for your info.

Hi all,

I already passed the NCLEX RN in Cali in 2009, didn't get license because no SSN. Now I have my SSN and trying to get my Cali license. They are asking for international diploma verification. Does the board normally asks for this for all international educated nurses? Can anyone help me how to to get this verification from the DFA? Thanks in advance.

Yes, they required that.. u need to get the form from them.. then sign and fill up bfore u submit to DFA phil together with your diploma from school and ched CAV.. then after that u mail it to BRN from DFA as sender they won't accept if its not from dfa and must be sign on sealed envelope. Hope that will help..

I have a problem too about the international diploma verification form. My aunt forward all the forms needed by the nursing school but said they cannot release all the requirements I requested because I am locking 1  form to complete which is the international diploma verification. Is there anyone who can help me where to access or get that form? If you have a copy , can you forward it to my email please. [email protected] Thanks

Search in allnurses.com the international verification form . There’s a link there where you can find it . Or go to CA board of nursing website

I have the same situation. My problem right now is who to send/give the International Verification form. I have the form and I asked my family member to give it to the DFA(Iloilo) and they told me to send it my school. My school said they don't know about this form. I'm confused now if Do I really need this form before I sent the documents to DFA Aseana. Anyone have thoughts or ideas about the process?

You have to send the form to your school from there they're going to send it to CHED for authentication and the CHED will send the papers back to your school and then your family member will pick it up and send them to DFA Manila. I believe it has to be an in person appointment at DFA.
