Funniest real orders you have seen in a chart?

To start things off, the best and funniest order I have seen on a chart, was in the discharge instructions for a trauma patient. It read simply

Darwin Consult

and was signed by the resident. Well the attending did laugh, but it was not the highpoint of that residents day.

For almost a month, we've had a patient whose family brought in her pet rabbit every day to visit. They brought it in a pet carrier, but the day staff wouldn't allow the rabbit out of the carrier to hop all over the room or whatever. So the family was VERY upset about that and complained to the doc. I guess he was sick of hearing about is, cause when I did a chart check at 2 am recently:

Please allow pet rabbit out of its cage (supervised) per patient request.

Who was supposed to supervise the rabbit? Did you need to get a sitter for the rabbit? How about placing the rabbit into restraints?

You know, when my patients want to do this, go ahead. I don't care.

But there was one event that I learned about on the news a few months back where a patient had a horse that he really loved. So his friend packed up the horse and brought him to the hospital. He actually managed to make it up to the floor where the patient was before anyone stopped him.

Yep, we have 2 docs that always do that as well. Manners. So simple, so powerful, so sadly endangered...

We just got this order the other day and had a good laugh. The order, Laculose po bid, hold for diarrhea. :lol2:

What's wrong with the above order? Just because something is a laxative doesn't mean you should expect diarrhea.

I was repriminded by my ADON for holding a laxative because the pt. had 3 large loose BMs. I was told I wasn't supposed to hold the laxative without a Drs order!:icon_roll

...on our MARs we have codes for "refused", "out of room" and "patient condition". I would think holding a laxative for 3 large loose BMs would be evidence of your critical thinking skills at work as a professional nurse, for "patient condition". You did the correct thing!!

I know, I guess my ADON didn't think I could make that judgement!!

This is what is called "nursing judgment". I would not give any more laxative either if the patient had 3 BM's unless your trying to clean them out for a test. Why is the DON even involved in something so minor as a BM? When I was director of nursing I had other things to thing about.

In LTC:

"May have occasional dippy egg"
