To start things off, the best and funniest order I have seen on a chart, was in the discharge instructions for a trauma patient. It read simply

Darwin Consult

and was signed by the resident. Well the attending did laugh, but it was not the highpoint of that residents day.

so do you have more?

I think the best one ive ever seen was on a med surg floor. The pt was confused and pulling out tubes. The order said "Do not restrain, do not let pt pull out IV". I just dont understand......................lol.

I don't think any male patient would elect to have their baloney amputated...

Not an order..but read this progress note other day for a 68 y/o pt being discharged who had complainted of erectile disfunction.

One of my favorites I see way too often:

Physical Therapy for progressive ambulation on pts (yes, I see it often) who have been paraplegic or quads for years

May ambulate PRN, this was in a chart of a resident that was w/c bound and could not stand.

A teaching hospital where I worked, one of the foriegn residents wrote "eyes and nose q shift". We all laughed for days over that one.

This wasn't an order but a return to the nursing home diagnosis-CATARACTS - probably caused by an impaction. I want this Doctor to see what's wrong with me...NOT!!!:chuckle :chuckle
