I think it is one form per year but you can always make amendments.

One FAFSA per academic calendar year, just add all of the schools you’re applying to on that one form.
I just logged into OPUS, and next to where it says application received, there is a small icon labeled “I”. When I click on it, it tells me to view my admissions decision on the portal, and when I checked the portal, there is no decision notification. Honestly, I’m 99% sure it is not an acceptance anyways. Good luck to everyone still awaiting decisions!
