What are your protocols on coumadin therapy? Is it standardized? Do you check PT/INR daily? Every other day?

The policy at the facility where I'm employed is to check INRs every Monday, Wednesday and Friday. Of course, if the doctor has orders for daily INRs, we'd check them daily.

Specializes in Critical Care, ED, Cath lab, CTPAC,Trauma.

Usually acute care they are done everyday.....LTC they are done less often

I just started my first job and it's in a Coumadin clinic. We have very specific protocol that calls for INR to be done at certain intervals depending on the current level. It can be anywhere from one week to four weeks but a lot of it is nursing judgment. Are you looking for something more specific?

Depends on the pt in the retirement home I work in. Some are weekly, biweekly and monthly. Rarely daily though.

This is exactly what we do except I work on a medical floor and our hospitals pharmacists dose it based on daily levels.

I'm on a primarily cardiac, high acuity step-down unit. Our Coumadin patients generally get a PT/INR every day or every other day, and if they're new to Coumadin, we refer them to our local Coumadin clinic, where there is a specific protocol. I think it's every day for three days or a week, then every other day, then weekly, etc.
