correct answer to Sickle Cell crisis question

the nclex review books are inconsistent with correct answer to sickle cell crisis question.:nono::nono::nono::nono::nono: the question is what do you do first if a patient comes to er with sickle cell crisis? answer to one book is administer iv fluid, another book says correct answer is administer oxygen. so what is the correct answer????????

I would say fluids because dehydration is what causes the cells to sickle..oxygen is obvi really important too but that is what I'd say...

actually i read a very go0d bo0k 0f MOSBY regarding sickle cell crisis.. y0u have t0 cho0se administer 0xygen 0n the 0pti0n if the 0xygen is a n0nrebreather mask because it delivers Fi02 0f 90-100%.. And if y0u have t0 cho0se between administering 0xygen via nasal cannula and IV fluids, g0 with the IV fluids because 02 via nasal cannula 0nly delivers 40% 0f 02..Hope it helps!!

Also, dehydration is the most common preventable cause of an episode.

Also, IV fluids for someone in a SC crisis (especially children) could help prevent a stroke. When somoene is dehydrated, the blood is thicker, increasing the chances of those deformed cells clumping together and forming a blockage or embolus.

i have encountered hundreds of sickle cell anemic clients way back in SAudi years, if the client doesnt have problems with breathing, and o2 sats reading is normal, the priority of care is IV fluids. next is O2 or pain killers. Management depends on client's status.
