Make sure the bell is "on".. Some can turn and if it is turned the wrong way, you will not hear anything. To check put your stethoscope on and tap, if you don't hear it loudly, turn the bell until you do..

Also palpate the brachial pulse prior to placing the scope on the arm. That way you'll know you're in the correct area. With practice you won't need to palpate but it helps when you're learning.

Earbud placement is key. The earbuds must be aimed forward to the tip of your nose, not pointed back towards the pinna (like they do on TV :) ). Anatomically, aiming them forward points them down the ear canal. You can rotate the headset pieces on your stethoscope until they fit your ears perfectly.

I used to have a problem hearing BPs even in the quiet, like you. But a new stethoscope made ALL the difference. I have a Littmann classic II now and I love it. Also note the earbud placement and having the diaphragm/bell the right position.
