bigeminy info please!

Hey -- I an RN with a wide variety of specialties in the past, and have been in the ER the last 4 years. One thing I have come across is a pt. presenting in bigeminy. Nursing school was a long time ago, and I wanted to check if anyone may could give me pointers or tidbits from school if you are a student, or your own experiences. I am wondering bout tx meds, factors leading up to it, or anything else anybody may know.

Thanks -- I am looking forward to any responses so I can more adequately care for my patients who present to our ER with cardic complaints.

Sometimes hyperthyroidism can cause ventricular arrhythmias, too. Maybe other hormones can play a part as well.

Since everyone is talking about strips... Anyone ever heard of Winke Bots (sp???).. There has been an argument on my floor about what exactly it is.. well i tried to google it and i can't find anything on it.. probably because I have no idea how to spell it!! Any one know what it is?

wenkebach (something like that) or 2nd degree heart block type I. Is where the pr interval before the qrs keeps on getting wider and wider until a qrs is dropped. Then it starts all over again. a great web site to check all these rhythms out with easy to understand explanations is richacls.com. Sometimes this rhythm is hard to spot because the pr lengthening can be subtle or it can be obvious. Don't see them very often on our floor so we get excited when we do. wooo!

Wienkebach, is another name for Second degree heart block type 1. It is a PR interval that gradually increases beat to beat until there is a missed qrs. Also known as Mobitz 1

I do UR and IC the last 3 years, so I'm not up on my ACLS, but do they still give Lido for this if symptomatic?

One thing that makes me look for Wenkebach (or however you spell it!) is that you get clumps of pretty regular QRSs with pauses in between. Of course that's similar to PACs, but with Wenkebach you don't have the PAC, but you do have the lengthening PR interval and dropped QRS.

Back in the 80's when I started doing this we seemed to see lots of Type II and III heart blocks, but I don't see them much anymore. I wonder if that's because of improving medical treatment and prevention of heart disease?
