As above. Can't think of a reason why you would be disallowed in Ohio from following an order protocol. Should be no different than any other order/protocol such as giving sliding scale insulin or adjusting a heparin gtt, etc.

Yes if there is a standard order /protocal for adjusting the dose but.....I always call the doctor tell him the lab results and what dose I am adjusting to.. never hurts to cross your t and dot your eyes so all on same page esp with drugs like blood thinners.

Nurses in my area of the woods cover the Coumadin clinic  but they are always NP's.

In the Alaska interior the Coumadin clinic is covered by pharmacists.
