has anyone worked as a CARE manager for Senior Bridge?

I got an email from a recruiter for care manager positions with Senior Bridge in my area. I've looked at their website & I think it's something I may be interested in. It's not home health care per se, it is more interviewing and documentation. The point is to prevent hospital re-admissions.

Just wondering if anyone can share personal experience with this kind of thing. Thanks!

I have had absolutely no case management experience. The trainers are wonderful! They are an email away and they are the ones that do the webinars. If you are stumped on something all you have to do is email them. If you are really stumped they will call you and talk you through it. Like anything new.. You feel dumbfounded when going to someone's house right at first, but I am sure things will improve considerably once I really get into it and understand exactly how I can help these people. They have a special resource department where you can ask for help with resources. You can only use Humana approved resources which they have available for you on. their site. If you find a resource to help someone that's not on their site.. You have to mail in a referral form with the info for it to be approved. The specialist depends on us out in the field to help them find new resources. I think the worst part of this job will be learning more resources out there to help these people and building a file of the resources.. But you are never alone. I also feel that the only advantage that nurses working for an agency have are being able to talk amongst themselves and helping each other where I am starting by myself and have nobody but the trainers to talk to. But in the end.. I am making double the money and Humana provides more than ample support and I can take as many or as few members as I want up to 35. They just increased their member load to 35 from 30. Most of these agencies are making their nurses take a full patient load. As an independent contractor I can take one or 35 members.. Whatever my heart desires because I am my own boss. It's a learning experience that I think I am going to love once I really get into it and learn all my resources

I have absolutely no idea what ashley is talking about in her comment about the specialist sending her the same list of clients and the specialist texting and calling her. I have NEVER HAD THAT PROBLEM. my specialist that's responsible for sending out a new list of possible members for me to pick from send me out a new list everyday. Some days there are some on the list that's close enough to my house for me to take and other days it's not. You also can take into consideration that even if you take a member an hour or 2 away from you.. If you pick up 1 or 2 more members in that area it would be worth your while driving time.

But I have NEVER had my specialist call or text me wanting to know if I would take a member,my specialist is so busy that she wouldn't have time to single me out to see if I want to take a member.

Do any of you work full time through Humana for Senior Bridge? The posting that I came across is through Humana, full-time, and doesn't say anything about "independent contractor" but I'm wondering if that is what it actually is. There are job listings for other cities that look exactly like mine except they specifically say "Independent Contractor."

I have looked on many Humana sites but have been unsuccessfull at finding the link to email them about becoming a independent contractor for care management with them. I spent the afternoon talking with people from Humana about this but no one knew anything about it. If you could direct me to the specific page it would be a great help to me.

It is a PUBLIC website where you can apply. Go to SeniorBridge Home Care and click on "join" then click careers under the drop down box.. Hope this helps AND GOOD LUCK!

Actually if you scroll down to the bottom of the page on SeniorBridge Home Care .. Look under " locations" and at the very bottom you will see "care management network" .. Click on that and follow instructions. You don't need to click on a specific location because that's their private pay program where they have offices. The care management network is nationwide and there are phone numbers listed for you to call a specialist for help. Good luck!

@jlminnc, LPN I'm glad to see your post here. I'm currently still working on completing all the requirements to submit. I have a lot of questions to ask but I can't post my email in here and can't PM. What can I do so we can talk? :)
