A) Medicate for pain around the clock.
B) Restrict foods high in fiber.
C) Avoid the use of stool softeners.
D) Encourage fluid intake of 2,500-3,000 ml each day.

what are you asking?
A client with chronic pain from herniated intervertebral disks is experiencing constipation. What interventions would be appropriate for this client?

Nursing intervention for constipations are:
High fiber diet
Increased fluid intake
Teach pt if possible to avoid drugs that cause constipation
Then maybe stool softner.
So now what did you think the answer is?
My first choice would be D (encourage fluid intake), but I wasn't sure about A (medicate for pain around the clock) for the chronic pain.

But the question is addressing the pt's constipation, so medicating for pain is not relevant to the question being asked.

And depending on the type of pain medication ordered (opioids), could actually be the cause of the constipation issue.
