New school nurse

Hello everyone! I'm a new school nurse with a background of OB and neonatal care...I'm a little bit overwhelmed by all the things expected from me at my new position. I have a ton of referral meetings coming up, not sure if anyone can help with those. I'm suppose to ask the parents about child's medical history. I know I have to ask prenatal care, birth hx, family hx, current conditions if any. I just don't want to miss anything. Is this pretty much all I have to ask? Thanks in advance!

I see no one has answered. I'm not sure what you mean by referral meetings. Maybe we call them something else in my district or don't have those.

Referrals for special education services is what I think you mean, Rcrcrn. Your district's department of SpEd should have a health/developmental information form for parents to fill out that covers all that information. Check with your school psych. Welcome to a wonderful job!

Welcome to school nursing! At my school there is a standard form for that & the SpEd department is the one who gives that out & has the parent fill it out. I am only brought into the loop if there is a medical need that needs to be addressed. Every district is different though, if they actually want you to sit down with the parent and do this, just ask the SpEd department for their form. If they look at you like you're crazy, maybe do an on-line search for a generic form :)

We also use a standard form/questionnaire for asking parents about their child's health history. If you private message me your email I'd be happy to share ours so you can get some ideas of things to ask. I'm also transitioning from a hospital position. I spent nearly 3 years in NICU so I know how overwhelmed you're feeling. I'd be happy to help in any way I can.
