Just a quick question really.
I have given IM Rocephin reconstituted with saline a few times until last night my charge nurse told me to give it with lidocaine solution.
My drug book had a small part in the nursing considerations when I looked it up that the drug could be given with lidocaine if the patient was sensitive.
Just wanted to ask people if they have given this drug and what they give it with?
no, not an idiot, just learned something new is all. I have given a zillion of these @ my last office when those who were really sick needed fast antibiotic. With the 500mg I believe it's 1ml and the 1gram it's 2.1 ml. Also, I learned that it tends to leave 'speckles' not mixed properly and if you roll the vial in your hands once constituted it helps mix it better tan than shaking it to mix... also the generic one I used to price check everything and get the doctor awesome deals on all the inventory, but the generic mixed horribly... thats when I started to realzie that 'rolling' was key.

I agree with those who have posted prior to mine. I have always given the Rocephin with 1% Lido. My STD patients in the ED actually asked for it if they have had it before. They seem to agree that the burning is less than with saline. Also, in the days of Punitive medicine saline is the standard.

Just check to be sure there is no lido allergy, and puhleeese reconstitute it w/Lidocaine! Before I was taught to do this in ER, I had many pts c/o how much the Rocephin IM injection hurt. After I began doing it that way, nary a complaint. BTW, I've been complimented many times over the years that I "give a good shot," so it wasn't technique. When I had to have a Rocephin IM injection recently at a Doc-in-the-box, I asked the MA to mix it w/Lido. His response? "I always do. It is a lot easier on my pts." When I got the injection I barely felt it.
now with the system at our hosp a Dr has to specifically order the lido, we used to just routinely use it, now because of our charging system we have to have an order. Or pharmacy in their audits counts it as as a medication error.
Moral of the story, is all pharmacists get it without lido, unless specifically ordered.

Absolutely. No excuse for not giving it without lido if there's no contraindication.
