Is this required by law in Texas? We generally here in Florida may either leave off the last name or we have the first name large and the last in tiny print. If there is a legal problem, the client can always subpoena the records to see who had his room from the facility. We started that because of nurses in military families and 9/11.

I've taken care of pts from Jails, Wife beaters, Alcoholics, Homeless people, Drug addicts, pt's with an extensive mental Hx and others that could jeopardize my safety/privacy if given the opportunity, In many states, police info is kept private to keep police safe, Shouldn't nurses be afforded the same precautions? If a pt has a problem with his or her nurse, its easy enough to find out their full name from mgmt, I don't believe any nurse should be made to display their last name if they choose not to do so or even have a last name alias for their badge. There was an incident a few years ago where a pt with slight cognitive problems kept calling a nurse at home, and eventually showed up at her house, the police had to be called.
