Ok...how could this happen? They must have been in unlabelled containers

Years ago, we used to carry floor-stock maalox in big brown bottles and the docs would order them left at patients' bedside for prn use.
We also carried floor stock peroxide in big brown bottles we'd use for certain docs' incision care.
Do you see where this is headed?

A mix up with one patient is pretty bad, but possible. Shouldn't the nurse have noticed something was wrong before giving peroxide enemas to the 5th or 6th patient. Or was she thinking, hmm a lot of gassy clients today. How long were they able to retain the enema? A whole new spin on "plop, plop, fizz, fizz"

Not to mention the people who have to go to the ER to have things removed from their exit-only orifice. (And they don't know how it got there.)

If you leave the lid off the bottle of peroxide, the extra O2 molecule evaporates, and it again becomes water - the off taste is the preservative.
I remember a doc ordering a 'high and tight' enema that included, among other things, peroxide. It was for a patient that had a BAD case of constipation. The enema wasn't harmful.
I gargle with peroxide - have for years - I'm still around.
