Nude underwear. Not thongs as they are easily seen.

Flesh-toned is the only way to go IMHO for wearing underneath white pants. There is a fairly recent thread about this in one of other sections. They do make lots of colors for women...if you have nothing against wearing ladies' undergarments for your clinical sessions, that is definitely the way to go unless your pants are very heavy material.

I never had an issue with solid white boxers or briefs under white scrub pants.

This.
When I had to wear white scrub bottoms I just wore some all white boxers underneath and had no issues.
I don't like regular briefs personally, and don't look right IMO under white scrubs, but they do make boxer/briefs that give the support of briefs if that is what you like but are longer than regular briefs.
You could also wear some white bike shorts or spandex.
I just realized this was in the men's nursing forum...disregard my comment and carry on.

All I know is that MEN in white scrub pants have NO worries compared to women in white scrub pants. Personally I think it's cruel and unusual punishment for anyone to be made to wear them. By CHOICE is a whole nother matter!
