What's you highest...?

So yesterday I was looking after this guy with rhabdo, who came in with a CK of 243,100 and I was wondering - what's the highest value you've seen? Not necessarily CK, any value.

An example, for example - I had a woman who had a glucose of 120.8mmol/L. I don't know how to translate that into the US scale, but the normal range is around 4 - 9 in a well-controlled diabetic.

Same patient, different admission - triglycerides 8.1 (0.0 - 2.0), cholesterol 22 (?

Highest K+? 7.6mmol/L (renal patient)

Urea - 48.6, creatinine 1.18!!! (same patient, normal levels in June this year)

Highest BP 310/145 (stroke patient)

Do we have any other contenders?

Just had a female today in clinical with a BNP of 1938. Could there possibly be any myocardial ischemic damage there? YEAH. Also had a guy who's FSBS was 23..and he was asymptomatic.

I had a patient with a serum glucose of 6. She died within an hour of the report.

We had pushed 3 amps of D50, and still no luck

We had a teenager with cardiomyopathy who came back from the OR post transplant with a K+ of 16... on ECMO and dialysis. She survived but ended up trached, with a colostomy, a rt leg BKA and severe muscle damage.

A preadolescent came in with a WBC of 111... needed apheresis for four days to bring it down without causing tumor lysis.

We once aspirated 840 ml of pure alcohol out of a 15 year old's stomach. No idea how much he ingested.

My son, while in MODS, had an alk phos of >3500 and a bili of 356. BUN of 29.8 and creatinine of 293. Four weeks prior to this he had a sodium of 112 and coma... precipitating his transfer to PICU. He's fine.

I can't remember what the ETOH level was but for some reason .270 keeps coming to mind...

glucose of zero.

I believe the pt was not on regular glu checks, but the chem 7 from the morning showed a zero, they called us, se confirmed with fingerstick, then she coded and died minutes later.

ETOH of 628 in one of our frequent flyers that never was below 300

You can use clip art from any graphics package, or you can use your own photo (doesn't have to BE you...) that has been downloaded from a web site or scanned and saved as a jpeg. You need to go into User CP then to Edit Avatar. It will give you options for choosing your image, and if the image is too big, it'll tell you. You'll have to use a photo editing program to resize your avatar until it fits. If you have trouble PM me and I'll help you.

highest glucose: 2749 (she lived)

highest lactic acid: 68 (he didn't -- in fact, they had to dilute the sample so many times that he died several hours before the result came back)

I had a patient that I was transporting (am an EMT as well). After assisting with 3 nitroglycerin, her b/p at the hospital was 270/144. I think I was sweating as much as the patient was at the time.

K level of 9.2!

Stupid MD put a patient on K replacement without ordering a BMP for several days. Pt had undiagnosed renal insufficiency and retained all the potassium.

She almost arrested, had to go on vent. I was called in to do hemodialysis. After 30minutes on the machine, her Twaves lost their elevations, HR went from 48brady to 75sinus etc.

Was pretty cool watching her improve so rapidly. Most of the time acute dialysis is boring

holy shnickity! 310/145 bp! wow. there was a guy today who had a BUN of 98, umm...saw someone iwth a low level, of platelets at 17,000.

I had a patient with Platelets of less than 11,000!! Range at our hospital is 140,000-400,000. This poor guy was not getting any better and the platelet level did not increase after many platelet infusions. He eventually went home with hospice.
